Justice Secretary David Lammy has confirmed that 12 more prisoners were mistakenly released in recent weeks, two of whom remain at large.

This follows 91 mistaken releases earlier this year, with the annual total rising sharply to 262.

The government attributes the errors to a paper-based system and staff shortages, and says a digital overhaul is underway.

Critics, including opposition MPs and prison inspectors, warn that the frequency of these incidents shows serious systemic failings, raising concerns over public safety and prison management.
